The Department of Neurology of Shenzhen Guangming District People's Hospital is a member of the Chinese Stroke Center Alliance, a member of the Neuroregulation and Functional Neural Network Collaborative Innovation Alliance of the National Clinical Research Center for Geriatric Diseases (Xiangya), and a member of the Ischemic Cerebrovascular Disease Interventional Specialty Alliance of Shenzhen Hospital of Southern Medical University. It is a designated treatment unit for stroke in Shenzhen. The Department of Neurology is a key construction discipline of the hospital and the fourth batch of key clinical disciplines in Guangming District, with strong technical strength. Since its establishment, our department has adhered to the talent training policy of "one specialty with multiple abilities" combining on-the-job training and external advanced study. We have sent many physicians to study in well-known domestic hospitals such as the First Hospital of Jilin University, West China Hospital of Sichuan University, Changhai Hospital, Shenzhen Hospital of Southern Medical University, Peking University First Hospital, and Peking Union Medical College Hospital. Now it has developed six clinical subspecialty directions: cerebrovascular disease (Stroke Center), epilepsy and sleep disorders, cognitive disorders and neuropsychology, neuromuscular diseases, neuroinfection and immunology, and neurodegenerative and genetic diseases. Our department also undertakes the teaching tasks of "Neurology" and "Psychiatry" for 3+2 undergraduate students in clinical medicine at Xiangnan University and Shaoguan University, and has presided over and completed multiple provincial, municipal, and national medical continuing education projects, with outstanding scientific research and teaching capabilities. Since the establishment of the department, the medical service capacity and service level have been quickly recognized by residents in the jurisdiction. The bed occupancy rate has remained above 100%, and patient satisfaction has remained above 90%. The hospital has a beautiful environment and standardized department management. Diagnostic and Treatment Equipment:

Our hospital has complete equipment and advanced technology for the diagnosis and treatment of nervous system diseases, with modern medical equipment such as 128-slice spiral CT, 16-slice spiral CT, 1.5T MRI, DSA, and color Doppler ultrasound. The functional department has specialized neurological equipment such as transcranial Doppler blood flow analyzers, video/dynamic electroencephalographs, electromyography evoked potential instruments, polysomnography/sleep EEG monitoring systems, and adult psychological assessment systems.

Personnel Structure:

The Department of Neurology is a key construction discipline of the hospital, with strong technical strength and a reasonable talent echelon. There are currently 18 physicians at all levels: 3 chief physicians, 6 associate chief physicians, 7 attending physicians, and 2 resident physicians, including 2 postdoctoral fellows, 2 PhDs, 2 PhD candidates, and 7 masters; 32 nurses, including 7 charge nurses and 25 staff nurses.

Number of Beds:

The discipline currently operates neurology outpatient clinics, stroke screening clinics, special clinics for headache and dizziness, and special clinics for epilepsy and sleep disorders. It has 2 wards with 66 approved beds and 80 open beds. The annual outpatient volume is 30,000 person-times, and the annual discharge volume is 3,000 person-times.

Scope of Diagnosis and Treatment:

(1) Cerebrovascular diseases (screening and intervention for high-risk stroke populations, intravenous thrombolysis with alteplase/urokinase for acute cerebral infarction, transient ischemic attack, cerebral hemorrhage, cerebral embolism, subarachnoid hemorrhage, cerebral arteriosclerosis, cerebral insufficiency, etc.), interventional diagnosis of cerebrovascular diseases (cerebral angiography) and treatment (bridging combined intravenous and arterial thrombolysis, mechanical thrombectomy/aspiration, angioplasty);

(2) Nervous system infections: encephalitis meningitis, neurosyphilis;

(3) Various types of epilepsy and secondary epilepsy, tic disorders in children and adolescents;

(4) Movement disorders: Parkinson's disease, essential tremor, chorea, hepatolenticular degeneration, dystonia, restless legs syndrome, etc.;

(5) Peripheral neuropathy: trigeminal neuralgia, facial paralysis, hemifacial spasm, herpetic neuralgia, multiple peripheral neuropathy, etc.;

(6) Spinal cord diseases: acute myelitis, subacute combined degeneration of the spinal cord, spinal vascular diseases;

Discipline Features:

1. Cerebrovascular Disease Subspecialty (Stroke Center) Introduction: It is a key cultivated subspecialty of the Department of Neurology. Many physicians have been sent to study in well-known domestic hospitals such as Tiantan Hospital, Changhai Hospital, and Shenzhen Hospital of Southern Medical University. It is a member of the Chinese Stroke Center Alliance, a member of the Ischemic Cerebrovascular Disease Interventional Specialty Alliance of Shenzhen Hospital of Southern Medical University, a designated stroke treatment unit in Shenzhen, and a unit on the Shenzhen cerebrovascular disease emergency map. The hospital's stroke center has a complete emergency green channel, and the emergency department can carry out ultra-early intravenous thrombolysis for acute ischemic stroke. Both the east and west campuses of the Department of Neurology can carry out cerebral DSA angiography and cerebrovascular interventional treatment (bridging combined intravenous and arterial thrombolysis, mechanical thrombectomy/aspiration, stenting for intracranial and extracranial arterial stenosis, thrombolysis and stenting for cerebral venous and sinus thrombosis, stenting for cerebral venous sinus stenosis) and other appropriate stroke technologies. Stroke rehabilitation equipment and facilities are complete, and rehabilitation nursing technology is comprehensive, which can carry out: early limb function rehabilitation, exercise therapy, occupational therapy, language function assessment and rehabilitation, swallowing function training, intermittent oro-esophageal tube feeding (IOE) technology, cerebral circulation therapy, neuromuscular electrical stimulation, transcranial magnetic stimulation, pressure injury early warning assessment and treatment, post-stroke depression intervention, psychotherapy, etc. In recent years, we have explored the medical-prevention integration work model for stroke, carried out community stroke high-risk population screening and intervention, promoted community rehabilitation of stroke patients, reduced medical costs and patient burden, and further improved the regional three-level prevention and treatment system for cerebrovascular diseases. For common and frequently occurring diseases, we have special clinics for headache and dizziness, and special clinics for cerebrovascular diseases. The cerebral function room can carry out various routine transcranial Doppler (TCD) items, mainly including: screening and diagnosis of cerebral artery stenosis and occlusion; diagnosis of carotid artery stenosis and occlusion, and judgment of collateral circulation; diagnosis of moyamoya disease and evaluation of surgical effects; diagnosis of subclavian artery steal and judgment of steal pathway; monitoring of cerebral artery spasm in subarachnoid hemorrhage (SAH). In addition, we also carry out: TCD cerebral blood flow microembolus monitoring, TCD bubble test (detection of right-to-left shunt in patients with headache, cryptogenic stroke, and syncope), TCD cerebral blood flow monitoring for cerebral infarction thrombolysis, TCD lying-standing cerebral blood flow monitoring (detection of cerebrovascular regulation function and autonomic nerve regulation function) and other projects.

2. Neuromuscular Disease Subspecialty Introduction: Key members have trained at Wuhan University School of Medicine, Peking University First Hospital, and Qilu Hospital of Shandong University. The work focuses on the diagnosis and treatment of difficult neurological diseases, specializing in the diagnosis and treatment of various myopathies, peripheral neuropathy, spinal cord diseases, motor neuron diseases, neurodegenerative diseases, and neurogenetic diseases. The subspecialty has a neuroelectrophysiology room and a neuromuscular pathology room. The neuroelectrophysiology room has 2 sets of Nihon Kohden and German SIGMA electromyography evoked potential instruments. Routine examinations include: concentric needle electromyography, neurography, nerve conduction, H-reflex, F-wave, repetitive nerve stimulation, quantitative muscle analysis, blink reflex, sympathetic skin response, somatosensory evoked potentials, long-term EEG monitoring, polysomnography, etc. These examinations play an important role in the diagnosis and differential diagnosis of neurogenic lesions, myogenic lesions, neuromuscular junction lesions, and paroxysmal diseases. The neuromuscular pathology room has pathological equipment such as frozen section machines and ultra-low temperature refrigerators. Routine skeletal muscle and peripheral nerve biopsies are carried out, with various histochemical staining and immunohistochemical staining, which are of great significance for the diagnosis and differential diagnosis of genetic metabolic myopathies such as muscular dystrophy, mitochondrial myopathy, glycogen storage disease, and lipid storage disease, as well as inflammatory myopathies such as polymyositis and dermatomyositis; at the same time, in cooperation with companies such as Shenzhen Aiwan Medical, KingMed Diagnostics, Oumeng Weiyi, and Concorde Global, we carry out diagnostic services such as gas chromatography-mass spectrometry for urinary organic acids, high-performance liquid chromatography-tandem mass spectrometry for blood amino acids and acylcarnitine detection, targeted gene detection for neurogenetic diseases, and whole exome sequencing.

3. Nervous System Infection and Immunology Subspecialty Introduction: It is a member of the Southern Central Nervous System Infection Alliance, a cooperative hospital of the Third Affiliated Hospital of Sun Yat-sen University in Guangdong Province, and a member of the Shenzhen Nervous System Infection and Immunology Branch Alliance. The hospital can carry out cerebrospinal fluid biochemistry, microbiological and cytopathological examinations, various routine immunological tests, tuberculosis antibody tests, T-SPOT.TB and other test items. The subspecialty, in cooperation with partner units, carries out microbial metagenomic testing, as well as antibody testing related to myasthenia gravis, autoimmune encephalitis, myositis, and immune peripheral neuropathy, further improving the diagnostic efficiency of diseases. Our department took the lead in carrying out research on optimizing treatment plans for myasthenia gravis, among which the optimized treatment plan for childhood myasthenia gravis was selected as one of the cover articles of "Pediatr Neurol", establishing China's leading position in the research of this disease. We carry out drug metabolism gene testing for azathioprine to guide drug application, and related basic research has been funded by the Shenzhen Science and Health Commission scientific research fund. In terms of treatment, in addition to hormone and gamma globulin pulse therapy, new immunosuppressants (tacrolimus, mycophenolate mofetil) and rituximab are also used to treat refractory neuroimmune diseases, with good results. The subspecialty's diagnosis and treatment scope includes: giant cell arteritis, various encephalitis meningitis, neurosyphilis, acute myelitis, neuromyelitis optica, multiple sclerosis, acute disseminated encephalomyelitis, Guillain-Barré syndrome, paraneoplastic syndrome, polymyositis, myasthenia gravis, autoimmune encephalitis, etc.

4. Cognitive and Neuropsychology Subspecialty Introduction: Composed of physicians who have been engaged in neuropsychology and dementia for many years. 2 people have passed the Guangdong Provincial Psychiatric Physician Transfer Training and have professional practice qualifications in mental health. They have undertaken the theoretical and clinical practice teaching of "Neurology" and "Psychiatry" in clinical medical colleges for many years, and have held multiple provincial, municipal, and national medical continuing education programs on "Standardized Diagnosis and Treatment of Organic Mental Disorders". They have solid theoretical foundations in neuroanatomy, neurology, and psychopathology, and have rich clinical experience in the diagnosis and treatment of cognitive disorders/dementia, anxiety and depression disorders comorbid with physical diseases, and brain organic mental disorders. In recent years, we have continuously explored the cross-integration of neurology, psychiatry, psychology and other disciplines to improve the identification, intervention, and treatment level of various neuropsychological disorders in general hospitals. The department can carry out transcranial magnetic stimulation for the treatment of depressive disorders, epilepsy, post-stroke rehabilitation, neuropathic pain, etc. The neurofunction room can currently carry out more than 20 commonly used neuropsychological test items, including Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Zung Self-Rating Anxiety and Depression Scales (SAS, SDS), Hamilton Anxiety and Depression Rating Scales (HAMA, HAMD), 90-item Symptom Checklist (SCL-90), Brief Psychiatric Rating Scale (BPRS), Scale for Assessment of Positive Symptoms (SAPS), Scale for Assessment of Negative Symptoms (SANS), Pittsburgh Sleep Quality Index (PSQI), Minnesota Multiphasic Personality Inventory (MMPI), Eysenck Personality Questionnaire (EPQ), 16PF Personality Test, Activities of Daily Living Scale (ADL), Social Disability Screening Schedule (SDSS), Raven's Standard Progressive Matrices, Raven's Advanced Progressive Matrices, etc.

5. Epilepsy and Sleep Disorders Subspecialty Introduction: 5 people have obtained primary and intermediate professional level certification in electroencephalography from the China Association Against Epilepsy, 8 people have participated in the "Galaxy Program" epilepsy professional doctor capacity building project training of the China Association Against Epilepsy, and 2 people have professional practice qualifications in mental health. We have special clinics for epilepsy and sleep disorders, equipped with advanced equipment such as central EEG monitoring systems, multiple video EEG monitors, American Embla polysomnography monitors, transcranial magnetic stimulators, and neuropsychological testing software. We can carry out routine EEG, brain electrical mapping, 24-hour video EEG monitoring, sleep monitoring, psychological testing and other examinations. The scope of diagnosis and treatment includes: (1) Various types of epilepsy and paroxysmal diseases: various types of epilepsy, paroxysmal syncope, convulsions, dizziness, abnormal behavior, etc. We perform video dynamic EEG monitoring, neuroimaging and genetic testing on patients to provide accurate clinical classification, differentiate epilepsy from other paroxysmal diseases, and guide rational medication. (2) Various sleep disorders: such as various types of insomnia disorders, sleep-disordered breathing, narcolepsy, rapid eye movement sleep behavior disorder, restless legs syndrome, periodic limb movement disorder, sedative-hypnotic or alcohol dependence, etc. We provide comprehensive diagnosis and treatment services such as polysomnography, drug therapy, psychological and behavioral therapy, ventilator therapy, repetitive transcranial magnetic stimulation, and acupuncture. The subspecialty works closely with the Department of Otolaryngology, Pediatrics, Imaging, and Psychopsychology, regularly conducts MDT diagnosis and treatment for complex and difficult cases, establishes electronic files for each patient, provides long-term follow-up services, and guides treatment.

6. Neurodegenerative and Genetic Diseases Subspecialty Introduction: Composed of specialist physicians who have been engaged in neurodegenerative and genetic diseases for many years. Key members have trained at Sun Yat-sen University, Peking University First Hospital, and Xiangya Hospital of Central South University. The work focuses on the diagnosis and treatment of neurogenetic and degenerative diseases, specializing in the diagnosis and treatment of neurodegenerative diseases, neurogenetic diseases, spinal cord diseases, motor neuron diseases, and peripheral neuropathy. The Department of Neurology is equipped with a fully functional neuroelectrophysiology room and neuromuscular pathology room, and the hospital is equipped with a central laboratory for molecular testing. We cooperate with medical testing companies such as Shenzhen Aiwan Medical, KingMed Diagnostics, Oumeng Weiyi, and Concorde Global to carry out genetic testing and molecular testing.
